For Scoot to complete your criminal background check, there are four steps we ask you to take:

  1. Download, complete and print this live scan form.

  2. Take your completed form with you to your local live scan location - we suggest checking if walk-ins are welcome, or if you need to schedule an appointment.

  3. Present your completed form, and have your fingerprints taken at the live scan location. Be prepared to pay approximately $90 for this service.

  4. Keep and upload both your completed form and receipt. Scoot reimburses up to $90 towards this expense in your first pay check, once you've started working.
